problem in my output data

Hi,
I'm having a problem with Concatenation(joining of my 2 fields).

I'm using this for the last name and first name of list of members, for my staff code (Department_Initials+unique code) and Etc.

Can someone help me to this problem??

If so, can you list the exact information in the fields of your external field so I can try to test it here?
