2003 CPT CODES ADDED. I have added the ICD 9 codes and V codes for OB/GYN with links to each one of the fields Surgery, Deliveries, Hospital Admits, and Consults. You can search for the ICD 9 codes and Cpt codes or use the old format I originally developed. OB ICD 9 codes may not have the 5th digit included but are followed by a + indicating another digit is necessary. The available 5th digits are included in the description eg (0-4) or (1,3) etc. Common language has been substituted in many of the ICD 9 descriptions and several codes not related to OB/GYN are included in the database. Use the FIND feature of the HanDBase program to facilitate quick location of codes, remember to check SEARCH ALL FIELDS when utilizing this feature. If you don't like the order of the fields, go to properties and click on edit field order. Suggestions welcome.
This ap is for the busy clinician who sees a number of patients a day and needs a convenient way to make sure necessary follow-ups are done with regard to lab tests, X-Rays, etc. Records include reason for follow-up, action taken, contact info, etc. Makes a good 'peripheral brain' for making sure all follow-ups are done (and a record that they were!). Version 2 corrected a problem with field lengths and added a 'delete' record button for each record. Version 3 removes the 'delete' button and gives the user the ability to save 'done' records to a seperate database entirely. This gives the user a more concise look at those follow-ups still not done. It also allows the 'done' database to be archived, printed, exported, etc. for future reference.

This is a logbook designed exclusively for maintaining a record of patients being followed up for acute Pain. It has addressed several deficiencies of the previous version. It is still not the ideal logbook but atleast it is simple & easy to maintain. There is no point in collecting lots of useless data which is not useful at a later date. What a trainee needs to collect to demonstrate proficiency in a particular procedure is different from what a consultant needs to collect. This database is just a skeleton - it can be personalised fully.

A value of -1 indicates that no datum was available for that antibiotic / organism combonation, while a number from 0 - 100 is the per centage of the isolates that were sensative to the given antibiotic for that organism. Some tables report isolates (from blood) and urine, while others just total # of isolates are reported.

Use the view for year 2002 to filter out all but the current data. Select the organism of interest and then use the page buttons to page through the sensativity profile for that organism - a little easier to see than the usual spread sheet view of the plain database.

Set a filter prior to doing a report. Since some puff fields may have a data value of zero; a missed reading, make sure you make a filter to exclude zero values because they will otherwise be included in the average calculation which will reduce your real puff average. I set the minimum value to something over 0. You can also set a filter to calculate through min and max dates. When this is coupled with the minimum value field filter, you can then generate a report for your readings since your last doctor's office visit that represents your true readings' average.

Is she worse on a monthly basis?), by time of day (could this pattern mean he needs a nap at this time?), by type of adverse behavior, by its severity and duration (Now I see that he's worst under these circumstances), by prior activity (oh, he most often tantrums if I take him away from the TV), by whether a transition is involved, and what it was, by how the child was approached before the behavior (Now I see that he screams if I approach him in a hurried manner), by various environmental factors (such as noise, lighting, clothing, the familiarity of people, locations, and tasks), by whether the child was tired or hungry, by interventions tried, and by the child's response to interventions (hmmm, I usually try hugging him, but he responds better to being given some space).

There are lots of Pop-Ups and there is one sample record included. In addition to the factors mentioned above, there is a place for noting the child's self-comments, as these can be enlightening. There is also a place for making general notes. It is NOT the purpose of this database to tell parents how to deal with their children. It is rather a venue for allowing parents to track behaviors, environmental triggers, interventions, and responses to interventions, so they can determine for themselves what is best for their child. The DB is fully user-modifiable. Parents should ignore any questions that they are sure are not pertinent to their child; for example, if clothing is never a problem for your child, just don't fill in that one. On the other hand, if you don't know what's triggering your child's behaviors, try filling everything out and see if patterns emerge over time. You may want to delete and/or modify any of the fields I've used, since they were designed specifically for my children. If you don't know how to do so, see the HanDBase manual, or e-mail me. I want this to be very helpful to other parents of kids on the spectrum, so please write me with feedback and/or suggestions.

It was designed for use with pump models 8617 (10 mL resevoir) and 8627(18mL reservoir), with a tubing volume of 0.0056 mL/inch. The pumps have a set volume of 0.26 mL with catheter acces ports, but the checkbox will permit the older variety with 0.23 mL.

This applet was originally based on a previous submission but I've extended it significantly to allow you to track your RM (Repetition Maximum) values for each exercise (1-6-12 RM) as you progress. This is helpful to maintain workouts at your maximum efficency. You can also add as many additional exercises into the exercise groups as you need. There are four database files that use Relationship and Linked fields to gain the maximum value from the applet. I have loaded the exercises database with the standard Body4Life exercises plus a few extra. You can add as many as you wish including cardio exercises which are also important in the B4L program. The Program database is the entry point where you define the workout for the day (upper, lower body or cardio) and build a workout for each muscle group in the upper /lower body group. I have left some data in the RepMax database so you can see the Relationship fields in action. Once you have a new RM value you can jump through the Relationship field to the RepMax database and add new values. The third database (exercises) contains all the exercises and related information such as tempo, description etc... There is now a 4th DB used to set the current date allowing the calculation of days since last exercise within a group. This is useful in balancing your exercises over the longer term. I also have developed a paper form that works with this if you're a little squeamish about taking your PDA into the fitness areas. ENJOY!

Client Database is a set of 17 databases that interconnect to give a very useful client management database for Social workers and alcohol and drug counsellors who have large caseloads. This database is easily edited to fit just about any counselling situation where detailed reports of the clients overall living and health concerns are necessary. Reports can be easily produced using Handbase's built in report and filtering capabilities.
To use this database I have my clients listed in a client category int the resident address book that are exported to a spreadsheet and then imported into the address database within Handbase. The Client database in Handbase uses the Address database for input through a series of db popups. The other databases in the zip file are all linked to the Client database.

The "linked" field type is used to edit each of the files. The number of "related records" is displayed next to the field to indicate there is related data in the file. The FamPersInfo database is primarily a "standalone" database that is used to maintain name information.
Allergy information is maintained in the FamAllergies database. The "relationship" field type is used to link to it from the FamilyMedical database using the "name" field. The allergy information database tracks individual allergies, test, provider, reactions, medications and notes information. A link is provided to edit the FamMedications file.
Immunization information is maintained in the FamImmuneRecord database. The "relationship" field type is used to link to it from the FamilyMedical database using the "name" field. The immunization information database tracks vaccinations, date of vaccination, followup date, provider, reactions and notes.
Medical history information is maintained in the FamIndMed database. The "relationship" field type is used to link to it from the FamilyMedical database using the "name" field. The medical history database tracks problem, when it occurred, treatment, physician, cost, pharmacy name, prescription number, prescription information, appointment information, location treated, surgery information, and followup information. A link is provided to the prescription database to allow editing of that file.
Medical insurance information is maintained in the FamMedInsurance database. The "relationship" field type is used to link to it from the FamilyMedical database using the "name" field. The medical insurance database tracks, insurance company, policy number, subscriber number, group number, plans, phone number, address, contact person, and notes.
The FamMedications database is used to maintain a list of medications. There are several entries in this file.
The FamPersInfo database is a "stand alone" database that is linked to from the FamilyMedical database. Its primary function is to record names; however, as a stand alone database, it may be used to record name, relation, social security number, driver license number, address, phone numbers, date of birth, race/sex, height in inches, weight in pounds, eye/hair color, health insurance information, doctor visit information, primary physician information, medical information, medications, and notes.
The FamBPPulse database is intended to document blood pressure, heart and respiration rates, and activity. Date, time, activity/exercise, BP systolic, BP diastyloc, heart and respiration rates, average BP information, average heart and respiration rates, and notes may be documented.
The FamExercise database is intended to capture exercise information. Date, time, exercise type, number done, start/end time, length of time, running length of time, average length of time and notes may be captured.

HanDBase and its application in the medical community:
Let me start off by saying HanDBase is a fantastic and very powerful relation database program for the palm pilot. Its true power comes from the fact that it is almost completely customizable and enables the user to link several databases together. Linking enables the user to jump from one database to another quickly and easily so that vital information can be accessed at a moments notice. I think this database is fantastic with only a few exceptions which I will outline later.
What I have done is taken the basic HanDBase program and created several databases to enable me to track patients in an inpatient setting. The database is set up with a "Main" database with several other databases linked to it. The "Main" database contains mostly demographic information about a patient and then linked to this database are databases which track: "VITALS", "IN/OUT", "CBC", "CHEMISTRY", "LFT", "ABG", "CARDIAC ENZYMES", "COAGS", "TO DO" LIST, "MEDS", "IVF/DRIPS", "STUDIES", AND "PROBLEM LIST".
Databases can be added or discarded as needed (very nice). I have found that it is quite easy to customize my databases for particular sub-specialty rotations that I have been on as well.
Please feel free to use these databases as you see fit. Post them, pass them to friends, etc.
Overall, this is the best thing Ive come across for patient tracking. The price is right, it is completely customizable, and it doesn't take a genius to figure out how to work it.
Anaesthesia Logbook which will allow the data to be accurately imported into the Desktop PC Royal College of Anaesthetists (UK) logbook by Hammond and McIndoe. To install the Indicant please unzip the downloaded file and copy four included files (Indicant.pdb, Gender.pdb, myBMI.pdb, LDLcalc.pdb) to your PDA. On WinCE machines it has to be in My Documents directory/its subdirectories.
When calculating the BMI make sure to pick the gender from selector - do not try to write it in. The BMI/Ideal wt calculators for now work only with metric values.

The submitted file is a ZIP file that includes the five database files needed to run this applet, as well as instructions located in a Read Me file.
The main purpose for this database is to log each Blood Glucose Test, detail list of Foods consumed, total number of Carbohydrates for the food consumed and on top of this calculate the total number of Bolus Units Of Insulin to program into your Insulin Pump. This is a great way of keeping a detailed log concerning BGL and Food that you consume (Doctors and Nurses do like the detail). The best part is all you need is a Palm, HanDBase program and a PC that you can extract the Log database to an Excel spread sheet which can then be printed or emailed to your Doctor or Nurse (detail information on BGL, number of Bolus units, food consumed, total Carbohydrates and any notes). I have created two databases one is called the LOG database, which will log all information that will be keyed into your Palm. The second database is called CARB-FOOD database (supplied with some data) which can be changed to your Food preferences. This just has a detailed description of your Food along with number of Carbohydrates for that food.
Example Calculating a Bolus For A Meal Using The Palm & HanDBase:
1. Check your Blood Glucose Level using your BGL meter.
2. Power on Palm.
3. Click on HanDBase Application.
4. Click on Modes, Open button
5. Click on Log database
6. Click on New Button
7. The date and time will automatically be time stamped in the log database. The UNITS field at the top of the screen will be the actual Bolus amount you will program into your pump.
8. Key in your Blood Glucose Level in the BGL field. Key in 165 (Example)
9. Press the ITCR field (Pop-up field, Insulin To Carb Ratio). I have it setup for 8, 10, 11 or 12). This can be changed by editing the Pop-up field). Press the Pop-up 10 (example).
10. Press the CRBS field (Database Carb-Food query). Press the Cheerios entry (favorite breakfast). This will bring you back to the Log database and all fields should be updated (CRBS, Food and UNITS).
11. You could then take the UNITS and program your Pump.
12. You now have a log of your BGL and what you actually ate. Their is a Note field that you can write anything that is pertinent to this entry.
Whenever you hot sync the database will be backed up to your PC. You can use the HanDBase desktop program to convert the Log.PDB to Log.CSV. I have written a macro that will take this file and imbed it into an Excel sheet producing a hardcopy listing (no big deal). You can email me at rinesj@simplexnet.com if you have any questions with these databases or want a copy of the macro. Two things are imbedded in the database which are used in the calculations the first is the Normal Blood Glucose Level (I have it set to 120) and the second is Insulin Sensitivity Factor (ISF) which for myself is 45. If you have different values then myself, you can manually change them by modifing the properties of the Log Database:

This is a completly revised (hopefully the final) version of my previous ITU logbook. It can be used for recording patient details, procedures, transfers and ward assessment of patients. It is quite simple to fill in and can be very useful during ward rounds for case presentations. You can personalise it fully.
A useful ITU logbook designed for all Critical Care trainees, especially those with an interest in an ITU career. Makes extensive use of FORMS for ease and speed of data entry. As well as basic demographics, it also allows detailing of progression of disease, and interventions, APACHE Scoring, airway, mode of ventilation, inotropes and type of Cardiac Ooutput monitoring and Renal support. Of cause procedures and supervision are also loggable! There is an "EVENTS" Tab, which allows a note on changes in condition. Importantly, you can keep a track of patients reviewed on the ward (but not admitted), and log the degree of involvment with the patients (from "Routine Care" to "Emergency intervention". This has been deliberately made as a "Logbook", rather than an Electronic Patient Record (of which there are several on this site).

Of cause procedures and supervision are also loggable! There is an "EVENTS" Tab, which allows a note on changes in condition. Importantly, you can keep a track of patients reviewed on the ward (but not admitted), and log the degree of involvment with the patients (from "Routine Care" to "Emergency intervention". This has been deliberately made as a "Logbook", rather than an Electronic Patient Record (of which there are several on this site).

This is a checklist of the Basic Clinical Skills listed in the Indiana University School of Medicine Third Year Procedures Log Book. I made this to use along with the iusm_procedures.pdb that is also listed on this website. This DB has fields entered for each procedure, what responsibility is required by the student and how many procedures need to be performed. This DB then has a field for you to enter the number of procedures you have completed which the DB then calculates the number that still need to be completed. Furthermore, there is a checkbox for checking procedures for which the required number is reached. If I can figure out how, I'll upload a revision which will automatically calculate if the required number is completed and check the box if appropriate. Please forward corrections to kswartz@iupui.edu - however, I will not answer support sort of questions (as I actually can't figure out how to get the iusm_procedures.pdb to work for myself correctly) nor will I accept any responsibility for screwing up your iusm_procedures.pdb - sorry, but use at your own risk. Some standard lipid profiles do not include LDL measurement - it has to be ordered separately. You can still estimate LDL level using this little applet and save your patient a buck or two : )
Known limitations: The Friedewald equation uses TG level to estimate VLDL and is not accurate for TG levels of 400 mg/dL and above.

Medbag was written using HanDBase version 3 and its companion form program. Its purpose is to provide a simple method to record what is in multiple first aid boxes based on "owner name." The "linked", "relationship," and database popup (DB popup)field types are used throughout the application. Editing of the "child databases" should be done through the "parent" database in order to maintain proper indexing. There are three databases.
Medbag is the primary database. It provides a place to record the owner's name, bag description, and notes. The "relationship" field type is used to "view owner items" and "# items in bag." The "linked" field type is used to link to the item list so that database may be edited. Tapping on "View OWNER Items" is the first step toward adding the inventory to the database.
The MegBagItems database is where the work is really done. It captures the item description using the "database popup" field type and links to the item list using the "linked" field type. Item description, purchaes date, expiration, number stocked, supplier information, and similar information is maintained here.
The MedBagList database is used to maintain a single item list of supplies to be tracked by the MegBagItems database. Cost information may be added to the application by adding supplier and cost information to this database and then use the MegBagItems database to calculate the totals.
Within the Medbag database, the "filter" option may be used to provide a list of items needing to be replinished, a list of items needed, and related information.
Setting up the internal linking for the "linked," "relationship," and "database popup" fields may be necessary in order for the databases to interact.

I use this as an in-hospital reference for conditions, diseases - anything I as a paramedic might need to look up when web access is spotty. The forms are geared toward lookup only - changes are made on my desktop and synced. This keeps me from making inadvertant entries while bounding down the road in an ambulance. This is the program I have used for the past few years during my evenings and weekends on call and I have modified it many times to best suit my needs. It allows you to keep accurate records of patient encounters while away from your office and transfer them to the chart later. I have a linked database containing prescriptions I frequently call to local pharmacies. Pop ups help you save time for the common complaints, responses and follow ups. The check box alerts me that I need to take some later action and I note what that is in the "note" field. The signature at the end of the record identifies me in the traditional format for dictation. Many of the fields are unused for any single call but they can be removed when you edit your hard copy.

One of the biggest time-drains in getting patients/clients medical equipment needs met is having the item signed-off on by the doctor and/or therapist (occupational, physical). Pediatric Advanced Life Support, based on the 2005/6 update. Algorithms include Bradycardia, Tachycardia and Pulseless arrest. Also included are the Rapid Sequence Intubation meds and drip rate calculators, PALS toxins, seizure, hyperkalemia. This is not a substitute for PALS training (obviously) and I can't promise the formulas are totally correct.

Built in to the checkbox/views system. Views: By Ward - sorts your active patients by ward. Alphabetical Acitve - Sorts your Active Patients Alphabetically by name. Seen Patients - Use this to export the patients that you have seen that day for billing. Don't forget to uncheck everything befor the start of a new day. New Patient - This is used to isolate the new patients which can be copied to a separte database and emailed to your partners in a group practice. Encrypt to maintain HIPAA compliance. Sign Off - Isolates the patients you signed off on, can be exported and sent to your partners to maintain their lists. Inactive - At the end of the day, move the patients you signed off on the the Inactive list. When the patients are readmitted, change them to active. Call for follow-up - If you type the words "Call for follow-up" which are also as a pull-down in the History Fields, this view isolates those records, if the patient needs follow-up phone calls. You can export to text and send it to your staff.

This software is perfect for the Medical Student or Resident in Medicine. It allows one to keep a record of all experiences one participates in during ones clinical experiences. This applet was originally designed for the new Florida State University School of Medicine. With the exception of a few minor fixes, this is the final product. It works one hundred times better than the old notecard and pen for long term recall.
This is a simple log that I must use in school to report what I see each day in clinicals. It is simply a tracker of complaints, age, assesment, if tests were preformed, procedures, and if I observed or preformed them. Again it is simple just to track my exposure. May be helpful for other students to track their exposure.
My Cardiology Group has been using the Patient Mobile Rounds Applet successfully for the past 4 years and it has revolutionized and transformed our practice! The power of it comes from the way we utilize a three check box system in conjunction with "Views" to isolate patients into categories. By using an "empty shell" identical database for group emailing, we keep "wirelessly synchronized". It also generates our daily bills. We have carefully chosen which fields export in each view. For example, we generate a daily bill by exporting relevant fields from the "Seen Patients" view.
By exploiting the fact that smaller HanDbase databases can be sent as email attachments, we keep all of our databases perfectly synchronized on our devices wirelessly, via a group email system.
Our Cardiology group has been using following HanDbase system for the past 3 years. Our database contains about 8,000 patients! Because it provides "point of care" information, it has streamlined patient care for us. By sharing critical patient information digitally, we all have up to date patient information right on our cell phones!!!
The system is simple to use and teach to others. I have taught it to physicians with no prior mobile computing or database experience. To keep it as simple as possible we have decided not to use forms, to avoid an extra layer of complexity. For Physicians using this system, data entry is almost self explanatory. HIPPA compliance can be maintained with the built in encryption feature.
ACTIVE VS. INACTIVE PATIENTS:
An “Active Patient” is a patient that you (or your group) are actively following. An “Inactive Patient” is one that you (or your group) is not longer following, however, you want to retain the information for when you need to follow the patient again. When you open a new record, the first field is a pop up field to delineate "Active Vs Inactive" patients (Active: Yes or No).
SEEN CHECK BOX:
After you seen a patient and deliver care, check the “SEEN” check box. Scroll down to the “Level of Visit” which has a pull down menu of frequently used CPT Codes (which you can edit if needed). Add information to the “History field” (there are 2 of them, each has a 2000 character capacity). If you are in a group practice, new information you added to the history field of an established patient can be copied and pasted to an email, to update your colleagues, so that they can copy/paste the new information to that patients History field in their database.
NEW PATIENTS:
Tap the “New Record” Icon to open a new record. Enter the patient’s name, MR# (Medical Record Number), SSN (Social Security Number) if needed, and DOB (Date of Birth) which is a Calendar, which then gives the patient’s age automatically. In the History fields, I recommend adding dates to major events for future reference. There is an extensive pull down menu for the first history field of common medical terms which could, of course, be edited to suit the users needs. Check the Seen box and the New box. Fill in the “Referring MD” and add your frequent refers to the pull down menu. The “Diagnosis” field has a pull down menu to add frequently used Diagnosis with ICD-9 codes. The “level of visit” pull down menu can be edited to suit you practice needs. The “AUTOMATICALLY ENTERED DATES” are set to when the field was first added and when the field was last modified. The “Seen 1st by” is helpful in a group practice.
SIGN OFF BOX:
If you a signing off or discharging a patient, check the “Sign Off” Box. Do not change the “Active” Field until after you do your billing (see below). You can create a list of the “Sign Offs” by going to the “Sign Off” view. Tap “Actions -> Export records to text. Choose a file name (such as “Today’s Outs”) and select a folder. You now have a text document of your “sign off” list which you can email to colleagues.
DAILY BILL:
The daily bill is generated by changing view to the “Seen Patients” view. This view only shows you the patients with the “SEEN” check box checked. Tap “Actions -> Export records to text. Choose a file name (such as “Dr. Smith’s daily bill) and select a folder. You now have a text document of your daily bill which you can email to your billing system.
SENDING NEW PATENTS TO A COLLEAGUE:
If you are in a group practice, you can email the new patients in database form to your colleagues so that they can "absorb" these new patients into their patient list. Go to the ”Today's New Patients” View. Tap “Action -> Copy records to…” then tap the “For Emailing” Database. Make sure the For Emailing Database was initially empty (Action ->Delete Records). The For Emailing Database can now be emailed to your colleagues as an email attachment. Encryption can be used for HIPPA compliance. When the email is received, the records from the emailed database are then moved to the main database on the receiver's device (Action ->Move records to...). This system can replace telephone signouts.
MOVING ACTIVE PATIENTS TO INACTIVE:
After you created your daily bill, change to “Today’s Outs” view. Open the individual records and tap the Active field (the 1st field) to change the "Yes" to "No".
FINDING INACTIVE PATIENTS AND MOVING THEM TO ACTIVE:
Change to the “INACTIVE” view. Tap Action ->Find. You can choose to search by patient's Name, MR #, SSN. Open the record and tap the Active field (the 1st field) to change the "No" to "Yes".
Note that all the RED buttons of the Form contain useful pull-down menus.

For simplicity, the Patient Mobile Rounds "Views" are carefully set up to implement various filters automatically. The user need not add their own filters, provided that the user properly used the check box system. For example, the "Seen Patients" View automatically filters the patients that were seen that session, to generate a daily text bill (by exporting to text in that view). The "New Patient" View isolates records to copy to the "For Emailing" database, to share new patients with your colleagues (and therefore keep your databases wirelessly synchronized). From the "Sign Off" View, a text file can be generated by exporting to text, which provides a list of patients to move from "Active" to "Inactive". This text file can be emailed to colleagues to keep your colleagues databases wirelessly synchronized. The "Inactive" patients are saved and upon readmission to the hospital, by changing the "Active" Fields pull-down menu from "No" to "Yes", the patient's record is now moved and available in the active lists.
Sorting:
For simplicity, the "Views" are carefully set up to sort the records for the user automatically. The user need not do their own sorting. For example, The "By Ward" View sorts the active patients by Hospital Ward. The "Alphabetical Active" View sorts the Active patients in alphabetical order. The "Inactive" View is also sorted by alphabetical order. Tap Action -> Find... to find an Inactive patient. When that patient is readmitted to the Hospital, by changing the "Active" Fields pull-down menu from "No" to "Yes", you change the status of that patient from "Inactive" to "Active".

We believe that our Patient Mobile Rounds Applet is the perfect example of people using HanDbase with NO PRIOR DATABASE EXPERIENCE, and creating an incredible database system which enhanced our medical practice to the benefit of our patients and our colleagues. Having a "For Emailing" blank database to email new patient records, and a check boxes that work in conjunction with various "Views", our databases are be kept "wirelessly synchronized". Information can be added to the database durning outpatient visits as well as hospitalizations. We strongly believe a mobile, "point of care" system such as this would be beneficial to all physicians who strive for excellence in delivering care. Physicians will immediately see the utility of having a handheld patient information management tool that streamlines patient care and generates a daily bill. This system replaces the "telephone signouts" and therefore, precious time is saved.
Note: Feel free to Email me with feedback or questions. Forms are still somewhat buggy with HanDBase 4.1 on Windows Mobile. I have developed a form for the applet but have not included it in the ZIP file. I believe this will be resolved in the next HanDBase WM update. I recommend you use backup software such as backup buddy, or something similar, because unless the new versions of Palm OS have changed--not all files get backed up. Also I'll put in a plug here for the ACR syllabus index, which is merely an index to the ACR's wonderful text collection, also found on the gallery...
Bare bones entry screens let you maintain an "interesting case log" from which cases can be pulled for lectures, conferences, submission for papers or texts or the like. You enter the name, and punch in the id# on the screen/keypad, most of the rest of the info comes from selecting popups, or is cross referenced in FINDINGSDB, which you must also download from this site for this puppy to work. If the findings aren't already in there, then you enter them,,BUT ONLY ONCE..instead of writing them over and over in a spiral notebook, only to loose (how many??) spiral notebooks over the years. With this program, you can filter
by date,area, exam, modality, or whatever for types of cases you would like to present. You may then read the list to your fileroom or jet off a copy of the filtered database through an IR adapter to a laserprinter. Your cases get pulled and you're off. I have nearly 1200 cases on mine and it takes up a pitiful 209Kbytes. We're not talkin' about entering images here--but the ability not to loose your valuable cases, and wear 2000cases on your belt???priceless.

This is a revised version of the School Nursing Log and Student MedBase applets. Three databases are included; School Nursing Log2, Student Medbase2, and Nurse's Report. The School Nursing Log is designed for School Nurses to keep track of daily student visits, while the Student MedBase allows for yearly student information to be stored for quick retrieval throughout the day. Records in the School Nursing Log can be copied and moved to the Nurse's Report Database for annual reporting purposes. A readme doc has been included which explains how the databases can be used most effectively.

1. This applet was written for physicians to be used by physicians who own HanDBase and PocketReport (with HanDBase add-on). The applet is designed to print prescriptions using the IR port of the Palm. Pre-designation of "favored" medications can be assigned. The applet allows for maintenance of commonly used prescriptions which then can be filtered by disease category, MD, or order and then "flagged" or marked for later printing to the memopad or printer using PocketReport. Limitations of the application are solely those of HanDBase and PocketReport. Forethought has been taken to facilitate movement between records in a logical fashion on the Palm Pilot. 2. "Seizure Log" is an applet/database that is designed to be a rapid data collection tool for use in recording observed and reported seizures. It is intended for both para-professional/allied health AND families with loved ones suffering from seizures/epilepsy. Has TONS of data in the pull-down menus&text boxes that can be used for rapid, accurate identification and recording of seizure attributes/manifestations for use in reporting the information to doctors/clinicians/caregivers. Features include automatic time-stamping of a seizure "start time" when you create a record so you can go immediately to describing the event once the person is made safe; it offers an easy selection of "seizure end time" AND automatically calculates the duration of the seizure. As a social worker with 38 developmentally disabled clients, I have observed a great many seizures over the years. It was not until my son was diagnosed with possible epilepsy that I began looking the world-over for a tool that allows one to accurately and rapidly record seizure events---I came up with nothing (there was nothing out there). I found this astonishing especially in light of the fact that diagnosing of Epilepsy is heavily predicated on family history and observation/reporting of seizure events, not to mention that treatment of the condition is even more predicated on the type of seizure based on descriptions of manifestations of the event. This tool was intended to eliminate that resource defecit. It may be the only tool/applet out anywhere like it. *Feedback&suggestions are entirely welcome&encouraged&updates will occur & will include relevant suggestions.

The Sunnybrook facial nerve grading system, first proposed by Ross et al in 1996, has a number of favorable aspects compared to the House-Brackmann system, including an easily comprehended single percentage (from 0 to 100) to grade the function of the facial nerve, with the logical incorporation of synkinesis, resting symmetry, and surgical adjuncts. It is excellent for communicating with referring physicians and with patients, and is sensitive to even relatively small change over time. The overriding disadvantage of this system is the requirement for writing down a variety of numerical estimations and then performing several calculations to arrive at the result. This disadvantage, which would be a major obstacle to its use in a busy clinical practice, has been eliminated by the creation of a simple HanDBase application which allows quick and easy use of the system in less than 1 minute per patient. I have incorporated this system into a busy private otologic practice with excellent results and feel that this has truly enhanced my evaluation of patients with facial nerve dysfunction. Interested physicians are encouraged to read Ross's original article in Otolaryngology Head and Neck Surgery 114:380-6 (1996) before using this application.

This is a procedure Log designed specifically for surgery residents. Has an associated form I had to upload separately for easier viewing and editing of records. Supports workflow of the surgery resident and likely will work for others as well.
Drop downs for can be edited and customized for: Attending, Role, Service, Year, CPT code. Also has 2 checkboxes: 1) Need to Dictate, 2) F/U (Follow-up). These are automatically checked with each entry so you can easily sort (with alternate views provided in the list view, not on the form) records still needing to be dictated, and by records marked for follow-up (I use them to mark records I still need to enter into the ACGME case log), but can be used to mark patients you want to look up later.
I included CPT codes for common surgeries just to facilitate entry of my cases later into the ACGME case log system since I hate looking them up there...you can edit this dropdown as well.
This program is great because I can instantly look up how many cases I did by attending, service, year, CPT and any combination of those.

This is a calculator for finding appropriate doeses of either Tylenol (acetaminophen) or Motrin (ibuprofen) for pediatrics. The Tylenol is based on 15mg/Kg and the standard 160mg/5ml liquid. The Motrin is based on either 5 (low) or 10mg/Kg (high) and the standard 100mg/5ml suspension.

I worked in an ER where we had a standing order for Motrin... 5mg/Kg for fever up to 101.9F and 10mg/Kg for temp over that.
